1.) I am 18 years of age or older.
2.) I understand my tattoo is permanent.
3.) I agree that the tattoo has been drawn correctly to my specification(s) & or spelling. Any discrepancy after I approve the drawing is solely my responsibility.
4.) I am not under the influence of alcohol or drugs and am voluntarily submitting to being tattooed without duress or coercion.
5.) I understand that there is a possibility of an allergic reaction.
6.) I understand there is a possibility of an infection.
7.) I agree to follow all instructions concerning the care of my tattoo and that any touch-ups needed due to my negligence will be done at my own expense.
8.) Variations in color and design may exist between the tattoo art I have selected and the actual tattoo when it is applied to my body. I also understand that over time, colors and the clarity of the tattoo will fade due to exposure to the sun and naturally occurring dispersion of pigment under the skin.
9.) I understand that tattoo inks and pigments have not been approved by the Federal Food & Drug Administration and that the health consequences of using these products is unknown.
(10.) I agree to immediately notify the artist if I feel light-headed, dizzy, nauseous or faint before, during or after the application of my tattoo.
11.) All of my questions have been answered to my satisfaction.
